Insurance/Billing

We are currently in-network with and/or accept the following Health Insurance:

  • Aetna
  • Excellus Blue Cross/Blue Shield of Western New York (including Blue Choice Option)
  • Fidelis Care
  • Medicaid (as primary insurance only)
  • Medicare
  • MVP (including MVP Option)

 

We do not accept the following Health Insurance:

  • Medicaid as secondary insurance
  • Non-HMO Medicaid
  • United Health Care (including Empire Plan)- beginning January 1, 2017

 

If we do not accept your insurance plan, we can still see you as a patient. However, you will be responsible for paying out-of-network fees. You may be able to submit a detailed bill to your insurance company for reimbursement. Please check with your insurance company for reimbursement policies and forms, as some insurances do not allow for this type of arrangement. ***The exception to this is United Health Care. We cannot see you as a patient if this is the type of insurance you have.***

If you are unsure if we are participating with your insurance, you may call your plan’s toll-free number to verify whether or not we are in-network.

Typical charges for services will be provided upon request, but they vary depending on the service provided. If you do not have any insurance coverage, Dr. Graff will discount her typical fees by 20% if the bill is paid in full at the time of service. This is to compensate for the decreased administrative work when a bill does not need to be generated and submitted to insurance.
