Insurance / Billing

Dr. Graff currently accepts Excellus Blue Cross/Blue Shield of Western New York products (including Blue Choice Option), MVP Plans (including MVP Option), Aetna, Medicaid, and Medicare, as well as multiple others. She does not accept non-HMO Medicaid. She does not accept Medicaid as a secondary insurance plan. As of January 1st, 2017, our office will no longer accept any United Health Care insurance plans (this includes the Empire Plan). If you are unsure if we are participating with your insurance, you may call your plan’s toll-free number to verify whether or not we are in-network.

If we do not accept your insurance plan, we can still see you as a patient. However, you will be responsible for paying out-of-network fees. You may be able to submit a detailed bill to your insurance company for reimbursement. Please check with your insurance company for reimbursement policies and forms. Some insurances do not allow for this type of arrangement. *The exception to this is United Health Care. We cannot see you as a patient if this is the type of insurance you have.*

Typical charges for services will be provided upon request, but they vary depending on the service provided. If you do not have any insurance coverage, Dr. Graff will discount her typical fees by 20% if the bill is paid in full at the time of service. This is to compensate for the decreased administrative work when a bill does not need to be generated and submitted to insurance.

Due to the complexity and expense of medical billing, Lake Country Family Medicine outsources to Priority Medical Billing. If you have questions regarding your bill, please contact them directly at 585-582-6234, or online at www.PriorityMedicalBilling.com.
